Welcome to the Cleveland Department of Public Health Birth and Death Certificate online ordering system. Requests will be processed from 10:00 AM to 4:00 PM on Monday through Friday.

 

Cleveland Department of Public Health ONLY issues: 

  • Certified Copies of Birth Certificates that occurred in the State of Ohio from 1908 to the present.
  • Certified Copies of Death Certificates that occurred in Cuyahoga County, OH (excluding the communities of Lakewood and Parma) from 1908 to the present.

 

GETTING A BIRTH OR DEATH CERTIFICATE

 

Birth Certificates 

  • The Bureau of Vital Statistics issues certificates for births that occurred in Ohio from December 20th, 1908 – present. 
  • For births outside the State of Ohio, please contact the state in which the event occurred. Click HERE to visit the CDC's website, where you can find contact information for each state.
  • For Birth records in Cuyahoga county prior to December 20th, 1908, contact the Cuyahoga County Archive.  For Birth records in any other county prior to December 20th, 1908, contact the probate court of that county.
  • Certificates for new births typically take 2 - 4 weeks to become available. While the Cleveland Department of Public Health strives to provide this service as quickly as possible, the information must first be received from the birthing hospital (or center) and uploaded into the State of Ohio records system.

 

 

Death Certificates 

  • This office only issues certificates for deaths that occurred within the County limits of Cuyahoga from December 20th, 1908 – present, excluding the communities of Lakewood and Parma.
  • For Death records in Cuyahoga County prior to December 20th, 1908, contact the Cuyahoga County Archive.  For Death records in any other county prior to 1908, contact the probate court of that county.
  • For deaths outside the State of Ohio, please contact the state in which the event occurred. Click HERE to visit the CDC's website, where you can find contact information for each state.

Social Security Numbers on Death Certificates
Per Ohio Revised Code (ORC) 3705.23(5): For the first five years after a decedent’s death, a decedent’s social security number shall not be included on a certified copy of the decedent’s death certificate unless that information is specifically requested to be on the certified copy. Relationship restrictions apply, and acceptable proof is required; for details, view the Social Security Number Guidelines and Addendum Form (PDF), effective October 15, 2015.
